ACPM would like to thank you in advance for your participation at Preventive Medicine 2009! We would like to ensure that your experience is a smooth, enjoyable, and rewarding one; however,  in order to achieve a successful process we encourage you to read and submit all signed forms and session materials in advance or by the deadlines set by ACPM.

Speakers & Moderators:

Please carefully review the Speaker’s Checklist and the Speaker Polices for Preventive Medicine 2009 for more detailed information. We encourage you to print a copy of the speaker’s check list to help you remember the different deadlines and materials that need to be submitted to ACPM for your presentation.

Each speaker and moderator must submit the following items:

Please return this information to ACPM by the marked deadlines. All materials must be sent to hbarno@acpm.org. Disclosure forms can be faxed to 202-466-2662. Please note that the MOC Questions, and the Session Abstracts and Educational Objectives will be included in the program as you submit them. These materials must be prepared by the session’s moderators and you should coordinate with your moderator before submitting these items. Please note that your name, title, degrees and institution will be printed in the program as they appear in the session materials or biosketch.
